A Report of Internal Biliary Tract Fistula with a Comment on its Complications


Abstract
Internal biliary tract fistula is not a common disease and has been only infrequently reported, Causes are cholelithiasis, perforation of peptic ulcer, external trauma, or operation of the biliary tract. Symptoms are high fever, jaundice, local pain, nausea, vomiting, abdominal distension, and others. The complications are gall-stone, ileus, .cholangiohepatitis, or obstructive jaundice, The present paper is concerned with 4 cases of internal biliary tract fistula, the 2 cases of which were complicated by arterial embolism.
The etiology was gall-stones in all but Case 1. Perofration of the duodenal ulcer was the cause in Case 1. All the 4 cases showed symptoms and signs of cholangiohepatitis with associated jaundice in all but Case 3, The liver function was impeded in the cases with clinical jaundice, Case 2 revealed multiple pulmonary emboli followed by infarction, and Case 3 showed the evidence of extensive myocardial damage on E.C,G, None of the present cases was complicated by intestinal obstruction which has been said to occur in more than 50 per cent of the patients with biliary fistula, Roentgen examination seems most helpful in establishing the diagnosis.
